SERVICES
The goal of Healthy Heart is to reduce cardiovascular risk factors in all community members, with an emphasis on those with diabetes, by providing case management and community-based heart healthy activities. We receive our funding through the Indian Health Service’s Special Diabetes Program for American Indians and Alaskan Natives. Community members who would like help in this area should come to Healthy Heart to speak to a recruiter, and sign up for services. We teach healthy living and monitor medications, blood pressure, cholesterol levels, waist circumference, weight, and activity levels.
